PrEP or PEP for free?

Foreigners can access PrEP or PEP for free.

Many steps involved if you want to utilize a dispensing station; not designed as a tourist entry point, as they are predicated on you already being in the health care system. This means registering and logging in to gov.br (identity layer) and e-saúdeSP app and its SPrEP component (health layer). Such requires a CPF that foreigners can obtain with some effort, and likely a local SIM for SMS functionality, uploading a recent HIV negative test, receiving QR prescription code, etc. The required teleconsultation prolly requires Portuguese language proficiency; on that step I am not clear whether it’s like FaceTime/Zoom or a series of form-filling. 

A better bet is to bypass all of the above prerequisites and attend via walk-in the staffed Jorge Beloqui Prevention Station at Metro República. Bring your passport and local address … CPF number if you have one. Rapid-test screening and dispensing provided. Much easier to use your phone translation app in person at point of care, if needed. If you are granted only 30 days of supply it’s not such a big imposition to zip back to República for refill.
