There is also a full service PrEP center in Metro Republica. 
I find that generally GPs are aware of PrEP and those who identify as gay/have men as their partners in their “leisure time” take it. Those who identify as straight, particularly those in relationships with women (who do not know of their dalliance) are unlikely to be on it, for fear of discovery. The coming rollout of semi-monthly injectable caboteglivir could well change that (although in some guys I do detect a “PrEP is for gay guys” construct that would also need to be overcome) 
Incidentally, I wanted to get a yellow fever vaccine, as I was later going to a country that requires it for entry. The process required registering for the health service (SUS), for which I now have an card, and the vaccine was free. I don’t know if this means I am fully enrolled in SUS, but it does seem that way. Also interesting that there was a whimsical poster up about the importance of calling transpeople by their preferred pronouns (how few countries one would see that!)

I’ve found that older buildings, which is to say every building in Copacabana although fairly few in SP usually have doormen with no biometrics and typically they have no problem with guests. In SP some buildings are all biometric with no human doorman, and  those have been easy too. The only time I had some trouble is from when there’s biometrics and a doorman and he’s, for example, watching to be sure everyone “applies” their biometrics. There’s a building right around the corner from Lagoa on Bela Vista like this. 
My favorite—and you make have to look a bit for these because they are only about 5% of listings—are those that have a charge for visitors (R$30 or R$50). With that I know that visitors are undoubtedly permitted and that at least the visitors I want to will have some interaction with and hopefully have to show their ID to reception (best system I have to say is in the Dominican Republic where reception holds the ID of the visitor until the host says it is OK to release it). Typically, if I’ve met a guy before, I just meet him outside and just walk in with him. Otherwise, I guess none of the other ones I’ve stayed with this rule have I “checked out” by giving a key to reception, But in any case, I’ve never been asked later for the “visitor money”.

Maybe I'm drawing a distinction where there shouldn't be, but I too always use this technique in the sauna, not with guys online, I suppose because there seems to be more diversity in pricing online. Although come to think of it, maybe I'm just allowing there to be greater diversity of pricing. Haha. That if I allowed them to state prices in the sauna I would be met with equal diversity (or perhaps even greater.
For those guys who do hire online, do you use the strategy of "beating them to the punch" of offering them a price rather than asking theirs?

The two examples above were particularly high, but I do find GP prices to be running far faster than inflation generally. Just since a trip in oct/nov Id have to say the average (for my type) in garotocomlocal has moved yet higher, from 350 to 400. In fact, especially considering the deteriorating exchange rate situation, 2025 may have involved similar GP inflation to the post-pandemic period, (when sauna prices jumped from 100 to 150 or more, a 50% rise )
By contrast my inflation benchmark is a chocolate cake I almost always buy at P.A.O. on Bela Cintra (highly recommended, along with the Fig and Walnut Bread) which did notch up from R$24 to 25 since november. But in general, the cake has risen from pre-pandemic at R$18 I recall, during which GP prices have approximately doubled (in my experience)
When average prices of the guys (or indeed of cakes) are quite low, I'm much more of the attitude of just throw a lot at the wall and see what sticks, which is kind of a fun attitude, and you discover some fun GPs you might not have guessed/ aren't entirely your type. With prices approaching 100USD, I'm probably not going to "try" 4 a day, as I have when they were below 50USD (2019?, 2024?) and be more discerning...

Hey @Grahm1eta. When you get there, it will be overwhelming. So make sure not to think with your little head. No matter what sauna you go into, there will be guys waiting for potential clients. They will smile and try to engage you in conversation. Most times, the guys are bold, and they will grab your hand and place it on their cock. Which if you are looking at a sexy stud, can catch you off guard. In my case, it made me forget what I was talking about to the person I went with. LMAO. Get settled and get a drink. While you are waiting, guys will try to get your attention. During this time, I look and see how many guys, if any, I want to go with and for what. During this time, since many of the gartos will try to get you to pick them. They will lift up their towels, show off what they offer. If you do not desire to spend time with them. BE POLITE this cannot be stressed enough. Because they are people that offer themselves as a service. But it garners respect from them, which can pay off in the end. Trust me, reading @Latbear4blk   , @Novarunner, @NoGagSuckerSF, @floridarob has shown. You might get more with a nice smile, not right now, and maybe later. However, if the guy has not approached you that you like, then go up to them. Discuss the price and what you want during your time with them. Some have suggested that you write on the phone screen shot it and save it so if there is any disagreement, it does not happen. You can point to it and prove what was agreed to. Which have saved us guys from paying for bad service. But in most cases, the guys working in the sauna want to give you the service you want. So you will come back and pick them. Or off them, aka take them out that night or during the day in some cases. Also, phones can be used to translate in the saunas. But not to take pictures of everyone in the sauna in general seating areas. But the best thing for me to tell you is to relax and take your time. Get there early enough to pick the best guys. Because some come early to get one or two programs, and they may bounce. Also read the guys that I referenced. They have a great deal of knowledge; they aren’t the only ones. But they have explored many countries and have great experience. It’s been helpful to me. For my previous two trips, and it still helps me as I get a third time this coming May. 

So I might as well turn this thread into my trip report…
Starting with a practical point, if you want a local number for your WhatsApp you can buy a Tigo eSIM at home. I started the activation at home, and I guess not surprisingly at some point it got stuck, but the surprisingly thing was it finished it’s installation on its own when I was connected to the airport WiFi. 
 
Armed with a local WhatsApp and a macdaddi edited greeting (mine, adapted, from my Brazilian one, was too informal for the mannerly Colombians, he said) I hit the mileroticos. You (or maybe I should just say I) think there are pages and pages of ads, but I guess they have to renew them every day or something so there are A LOT fewer than I thought. And not great diversity in types. I would say no more than 5% are over 80 kilos, for example. Not that it mentions their weight (or, vexingly, their height) in the ad. it shares an annoying feature with garotocomlocal that there’s no organized fashion of presenting theInformation, as for example, rentmen. Dick size is typically mentioned, except probably when it is unremarkable. Unfortunately, over the course of a couple days, I made it through the ads back till mid November, contacting about 10-15 that Interested me. About 2/3 responded.,a little higher percentage than Brazil, Most did not meet my height threshold (180cm) And a couple more stymied by my thicket of questions about what I’m into. Two Survived, One of which I told that I would be moving hotels later in the day to one where I could receive guests but he said that if we could meet, then he would pay for the hotel because he was desperate for money. I think I managed to annoy Fercho, although the feeling was mutual. He kind of lets the guys out like a leaky sink one drip at a time, and even though my first requirement is 180cm he kept sending me ones that weren’t. Actually I thought he had ghosted me but in fact he just sent me a whole bunch of messages, just now. He speaks so fast I have to get someone to translate them. He does have a guy at only 175 that I can’t seem to forget about, and since many of the guys claiming 180cm are actually 175, I may have to go for it. 
Macdaddi was kind to refer me to Otoniel, the polite term for which could be a curator. He is super super nice.  Met 2 in his “stable,” nice guys, friendly, good at sex, but neither great. At least for my type, the “stable” is not large/he has a “shallow bench” however you want to put it, because after the two today he had never met before. I don’t know where he is finding them. Yesterday’s guys were very nice if a bit lackluster in sex. Whereas, the first one today spent the first half hour taking a shower and finding straight porn that he liked, but finally when he turned (at least some of) his attention to me, he did give a really great fuck for about 15 minutes but then said something about his rate being 250 (it was 200) and if he went on, it would be another 250. Various pics, maybe 10 guys, were sent. I asked if any were at least 1.80 and muy bien dotado. Yes, at least for the ones I’ve contacted (A couple of guys said 250) Twinks or bottoms might be less than 200, but that’s speculation; there seem to be more of these than garotocomlocal. The guys rarely look muscular. And of the ones I’ve met, 2 of 4 were notably skinnier than their pics, which is disturbing to me, both for aesthetic and moral reasons. I don’t think it’s because they’re using Ozempic.
in fairness, I should mention the other side of the coin, which is that presumably far more Brazilians are using steroids, although I would say the “overdone” roid guy has declined markedly since the pandemic, and that oil-injection thing (with those freaky trapezius faux-muscles) has all but disappeared. I stayed in a Hotel Colonia Casa Conde or something to that effect hotel on the first block of the Conde, main street of the Colonial Zone; cheap and cheerful. The neighborhood seems to deteriorate quite quickly heading uphill (north) so that many posters advise an Uber for the 3 blocks to Juan Binbin's (house of boys). I walked but the last block did feel a bit unsafe. Not sure if things go downhill so quickly to the west. I wasn't really that into Manny's guys (yes, they have enormous dicks, but typically lacklustrer body and face, in my opinion; of course now, 4+years later, I imagine he has a whole new stable.) Do know that is a real differentiator in Medellín. Despite a growing foreign tourism boom, they are just catching up with demand for English language services. If you are comfortable with Spanish, I can make some introductions. Your move. 

@Chucky is locally based in Medellín and offers sound advice and insights. As I have posted here in the past, the scene in Medellín is a tougher nut to crack for the first timer. From the trip duration and expectations you described, I am going to suggest you organize a couple of encounters with Rent.men rather than chase down the local happenings. Once you are in contact, ask those guys if they can recommend other scorts with your desired attributes.  Most of those escorts have connections with the guys they do threesomes (and more-somes) with, and will have short list of recommended providers. 
Since the Mileroticos listing left you cold, I would also suggest checking back. Many of the scorts on offer travel around Colombian cities, or even do the circuit of Mexico-Panama-Colombia. So new listings are frequent, plus the Mileroticos algo is weird, does not always show the newest listings. Try searching the city listing using heights, starting with your minimum of 180cm.

MacDaddi is THE expert with the answers. However, 2 the best of my knowledge, in medellin there is also el silon rojo club, and about 6 more saunas and bars.
 

So, you are correct in some ways, the saunas in Colombia are not really about hiring, although some of them have guys for massage that can be contacted reasonably quickly. The saunas in Colombia are busier socially and in line with Brazil around the end of the working day. I've always enjoyed the saunas in Medellin but its a very different vibe from say 117 or 202 in Rio. In respect to your type, again not necessarily so easy in Colombia, in general the men are less tall than in Brazil average in Colombia is around 177cm but they are well endowed so that shouldn't be a problem. 
If you are around Poblado and go on Grindr or Scruff there will be some guys who are available for hire, and will often message with their services.
Bogota the same, but you do have as you mentioned the St Moritz option, but the last few times I was there, the guys were definitely more twinkish. 

@Canadianbtmguy Haven't used them recently but they are still in service. I suspect that they have moved locations again, but that is just a guess.

I'm trying to pick apart whether they just seem safer (which certainly they do to me too...) or if they really are safer. Certainly they are safer with regard to what various possessions you might have in a hotel or airbnb. But as far as your personal safety, immeasurably more valuable. That's something that they wouldn't leave until they were sure you were ok. I wonder what would happen after many hours, a shift change etc. and  James was nowhere to be found if they would hold him there. I'm curious how James's assailants dragged him passed reception, out onto the street and into a car. One of the few motels I've been to in Costa Rica had the garage attached to the room (for discretion); a kidnapping couldn't have been easier.
In some ways, rather than a motel, it seems a cheap hotel at which you are not staying (with paper thin walls) would be a better choice. On a long layover in Bogota (on my way to Brazil, I confess...) I met 2 guys (separately) at a hotel within walking distance of the airport (Estadio El Dorado, 130k cop for 4 people). ALthough it was early afternoon and few guests were in the hotel, I would think that in the evening the small room with shared bathroom would either inhibit violence or quickly reveal it. (Granted it would also reveal any moans of pleasure so perhaps the shy or inhibited may not like the idea....) Just an idea...
