if you have hotel reservation and/or ticket attach to application. I usually attach only one reservation , even if it's for just for one night and I'm going for a week. I wouldn't worry about bank statements or insurance unless you have copies of those easily available.

If you apply on that website , on top of 30 $ visa fee you will be charged 6 $ administration fee.

After several years of applying for visa on internet , last time , in July I decided to go for visa on arrival in Phnom Penh. It was extremely smooth process. All they wanted was passport and 30$, no other documents asked for , neither photo was required. It took probably 90 seconds. Visa was just small piece of paper, no photo , nothing. Don't lose it as they may want to ask for it on departure although they did not ask me . Not sure if in Siem Reap is equally smooth. Others may chip in .

I do not attach those documents, and my visa is always granted. I dont recall a $6 admin fee either. I arrived yesterday from Bangkok. ATR, so sat at back to be first off. However, as often, we were only plane for 2 hours. I had filled in my e-arrival form, as directed. Although there was an immigration official, very quick, I was first pax, and I even got an email saying that I was legally in the country. Rather good.

I submitted all the required docs; took me much less time than I had expected. 1st response said not to expect early confirmation, because my arrival date is nov 9. Nevertheless, 48 hours later visa arrived. (-:

Same here. I did not submit any of the documents. Treated it as 'optional'. I did pay the $6 service fee in addition to the $30 visa fee. Received the visa within 24 hours. Completed the arrival card online in advance as well. On arrival in Siem Reap was all automated. Fast and easy process.

Not sure about that. I couldn't check out without paying it. Wasn't an option.

since Keithambrose said he did not pay that either my strong suspicion is that it's levied only on applicants from certain countries . I always pay that and jimmie50 says now he did as well

The $6 fee may have been included in the plane ticket, perhaps depending on the airline.

This is how Japan collects it's 3000 yen exit fee.

I suppose so, I got my ticket, Bangkok Airways, 3 months before, and I had no idea if anything like that was included. You put in flight no, when you apply for visa, so I suppose the $6 could be included, but its asking a lot!

it would be bit strange to include visa administration fee but not include visa fee itself but what do I know?

I've never handed over the exit fee at any airport in Japan and didn't even know about it.

It's not a visa fee but it is fee administered by the government.
