There are several massage shops that send out Thai language Line blasts or updates and they use "gay" with the same sense as we do.  For example, VCK - will say who is "" เกย์" ("gay") and who is "ชายแท้" ("chaai-táe"="real man"=straight).  They will also say if someone is top only, does it all, or does absolutely everything. And these gay guys are not ladyboys or feminine. 
There are other shops that will identify guys as G or M for "gay" or "man=straight" , so you will see GT or GT&B or MT or the rare elusive (but prized) MT&B.  Crowns Club is another that identifies guys as "" เกย์" ("gay") or  "ชายแท้" ("real man").  And the list goes on and on.  
Also, in my experience just in conversations with local guys there is no direct link between "gay" and ladyboy.  We freely use "gay" in the "gay Thailand" sense.  To confirm my understanding, over breakfast I asked a young lad from Laos if "gay" would really make people think katoey, and he thought not, but, again, not a statistically valid sample.  (Over breakfast = another of those uninvited L/Ts.  )
It is true that most of the gay gogo bars have a drag element in the show.  But you find that in gay bars in NYC and Atlanta and elsewhere.  Go figure.
At bars, I always ask guys if they are "gay" ("เกย์"), and at massage shops if I'm selecting from a lineup or photos I always first just tell the manager it has to someone "gay."  Again, no hint of ladyboy.  Way back I used to say I didn't want straight/top only - because that's all I cared about - but this way of asking-by-exclusion doesn't work as well as just saying gay.  Actually I say I want someone "gay does everything" ("เกย์งานครบ ได้หมด").  Can't go wrong with that.

Regarding Q2, the Patpong Soi 2 bars will have full exposure with engorged appendages at some or several points in their shows. Most of the time, the performers will come down from the stage and circulate amongst the audience for a few seconds' grope and a 100-baht tip. You can say "No, thank you" if you think 100-baht (3€) is ridiculous for a very brief handling of a sausage tied up in rubber. If you want something more aesthetic rather than crude (erotic is probably too much to ask for) Moonlight's and Jupiter's shows might fit the bill, but there will be some show items when drag lip-sync performers take centre-stage.
Two bits more of explanation. Why do I say erotic is probably too much to ask for? Well, the performers are mostly straight. They may do nudity, even as a group, but touching another straight performer on stage in a truly erotic, sensual way is either beyond their instincts or inclination. They just don't do or maybe don't understand homo-erotic. How many of us would really know/want to do erotic with a woman even in a show act? Why can't they be trained? Bar owners can't really be bothered, especially since the boys move from bar to bar. Anyway, bar owners are policemen's wives (at least according to received wisdom), not show directors. Second bit of explanation: I noticed you used the word "gay" in Q2 in a certain way. Avoid the use of this word (as you understand it) in Thailand. Thais tend to understand "gay" in a different way. They conflate it with katoey and cross-dressing. So if you use "gay" in your way, you will likely meet with cultural misunderstanding. E.g. if you ask to be shown to a "gay show", you will get drag.
Regarding your Q3, ah, I see. You want homo sex 24/7  Unfortunately, not much to do in the day, in this respect. There are possibilities in the afternoon, say, after 3pm, in the saunas or massage parlours.

Let me try to help with some of your questions.
What/whom to avoid? Touts, touts and touts. They'll make a beeline for you once you look like a newbie. Mostly found in the Silom, Surawong and Patpong areas, they'll want to show you where the bars are, etc. Don't let them. Don't even answer their questions or make eye contact. Pretend that the only language you speak is Belorussian or Fulani and cannot understand a word of what they're saying. Keep walking on.The bars and massage places are not hard to find by yourself. Thailand is pretty open about sex and related businesses, and these establishments are not hidden in deep back alleys where tourists would never dream of entering alone and so might need someone to show them the way. They're on the main street! They have bright blinking neon signs.
I would assume you have some sort of gaydar. Turn it on. Spot the gay guys and see where they're going. Follow.
How much is an off? It'll vary from one place to another. Just ask the bar captain as to the bar fine and the desired boy about his own charges. They'll be upfront about it (especially the Vietnamese!)
How much is a tip at the table? I assume you're indicating a situation where you've had a bar boy sit with you for more than 3 minutes but you really don't want him to continue sitting there, or he doesn't want to continue sitting there, so you part ways. 100 baht would be fine, that is in addition to the drink you've bought him (which he might have hardly touched, but that's not the point).
A quick short visit like you have planned likely means you'll spend nearly all your time in touristy areas. The bars and massage parlours in these areas see mostly tourist patrons*. This means there is no working week to affect the pattern of traffic, so whether you go Saturday, Sunday or Monday won't make much difference.
*On this note, I'll add that first-time farangs may make the mistake of thinking that the Asian-looking patrons of bars and massage parlours are Thais. While that may be true in some massage parlours, in general the Asians are tourists like you, from other neighbouring countries. Some will outspend the farang by a mile. They are now THAT rich!
Best shows? It depends whether you're looking for Magicians pulling rabbits out of hats or nude girls spitting darts from their vaginas. Could you be more specific?

Most of the bars on Patpong soi 2 (Hotmale, Lucky Boys,Dream Boys, Fresh, Screw Boys) and the Banana Boys on SIlom soi 4 will have some lean muscle chaps amongst their off-able crew. Ditto with the selection from Jupiter and Moonlight, though maybe a bit pricier -- these two bars think they are classier.
Thais, Cambodians, Vietnamese, Lao and Burmese are almost all uncut. In fact, hey, most of the world are uncut 
Speak English? Keep your hopes low, real low. Consider this, how many farm boys in your country, hustling in the cities, speak Thai?

Having a stroll down Walking street on Halloween night I was very surprised to see a big billboard about where soi Diamond starts from Walking street, proclaiming "Boys Yes" and photos of half naked muscular men. If I understand the billboard correctly it is the lady agogobar "Yes" in soi Diamond, one of the small side sois to Walking street, that after 2 am now turns into a boy showbar. 
One of the many barkers on Walking street with their laminated papers about ping pong shows etc, noticed me looking at the billboard and approached me claiming "Have boy show! 500 baht entry!". I have no idea if he really knew anything about it and I didn't want to talk with him, and the quoted price is most sure very inflated. I’m in Pattaya just now - arrived yesterday pm  I intend to check out Toyboys tonight. Last night I visited Winner Boys in Sunee, not intending to off due to being rather jaded and tired after 24 hours “on the road”. After sending all my family to nap, I visited Prince(#3) again. I have no recollection of that massage which means I enjoyed it but it was not exceptional. What I do remember is the price change. 1h Oil 1100 and minimum tip 1200. I'm almost sure 2 weeks before that both were cheaper. After my family woke up we all went up Mahanakhon building. Leg 5 - Singapore - where are the shirtless gay men dancing?
Arrived to Singapore Friday after noon for 4 nights. This was my first visit there. My dad joined my trip on this leg which had a positive and negative impact on my trip. On the one hand, I stopped paying for accommodations and generally everything except my own expenses when I was out at night. On the other hand, I shared the bedroom with my daughter and lost my privacy and ability to host. Although I did not feel comfortable hosting ever since my mom came even though we were in separate rooms. 
Hotel review - Ascott Orchard - 2BR apartment (450 USD per night): Toto, I don't think we are in Kansas anymore. Well the price speaks for itself. This is not Thailand and accommodation is expensive. Taking that into consideration, apartment was really well decorated and we enjoyed it a lot. We stayed in that apartment for 2 nights. 
Cruise - I visited this gay sauna during my first and second night in town. This was actually a pleasant surprise. Place was not packed at all both times I was there. But, a fair amount of the crowd that did attend the place were very much to my taste and I enjoyed this place on both of my visits. 
Taboo club (#1) - This is Singapore's current main gay club excluding some circuit rare events. I visited this place on Friday, my arrival day. I was very disappointed to find out that everyone is not only wearing their shirt, but also dressed really elegantly. I came wearing party wear and felt a bit misfit. I don't do elegant. Music was more pop than house so I left after an hour. 
Tantric bar (#1) - located opposite to Taboo this place is where all the mingling happens. Gay mingling and socializing is not my cup of tea. I leave that for the Americans lol. But having no other options I came there, and the atmosphere is vibrant and friendly. I will say that although Singapore nightlife was a little boring compared to my expectations, Grindr was boiling. Maybe it is that way as a consequence of that fact. 
Grindr hookup (#7) - At the bar, I get a text from a local couple. I try to avoid sex with couples but so many of my principals fade away when the guys are hot and I'm horny. Met them at their apartment for a surprisingly hot and fun interaction. Was there for a couple of hours, first hour was drinking and second hour was me and the Asian taking turns on the white guy which was awesome. Hotel review - Marina Bay Sands - 2 Interconnecting garden view rooms (390 USD per night per room): Well, this place is a factory. A very nice one, but still a massive holiday monster with a Vegas vibe. Pool is what I expected - beautiful. And it was nicer than I expected in terms of calmness. Rooms were huge, clean and a little outdated. It is an overwhelming experience to stay at this massive monster. It is definitely not worth 4 nights since it's massiveness is a bit tiring and charmless. As an experience I do recommend 1 or 2 nights.  
Third day in Singapore was dedicated to enjoy our new pool and the hotel surroundings of mall and beautiful gardens. Hotel gym was impressive too with views from the 55th floor. 
At night I was still very tired from the night before and decided to have a chill night at Tantric (#3) with a beer and go to sleep. Tantric was very calm that Sunday eve and I enjoyed nice conversions with some locals. After about an hour, 2 guys entered the bar. I recognized them both from Grindr since they texted me regarding a threesome and I ignored. Not because I didn't want it, but I was unsure and when I'm unsure I prefer not texting until something changes which is usually when I'm more horny and/or drunk and/or see the guy in person and know how he really looks. Upon entering both guys sat next to me and started flirting. One guy is local and is my age and shape, the other is a tourists in the muscle daddy category. They were good friends but not a couple. Leg 4 - Chiang Mai - The laid-back leg
Arrived Chiang Mai Tuesday After noon for 3 nights. this is my first visit to this city. This leg was probably my least eventful. 
Hotel review - Le Meridien- 2 interconnecting rooms (110 USD per room per night): The location of this hotel is great. Rooms are relatively large and comfortable. I can agree with @DivineMadman and @PeterRS impression of this hotel. This hotel has very little character and is more like a massive business hotel. But, service was great, rooms also and hotel Italian restaurant was high class. Breakfast was a bit too crowded and of fair quality but not what I expected from that brand. 
Adam's Apple - After dinner and a visit to the night market, I went out to explore a place I had some expectations of: Adam's Apple. That bar is nice, professional and friendly. It was also rather empty that night and I was 1 of 3 customers. About 20 guys participating in a rather nice and professional show. That place was entertaining but guys on display were on the twinkish side so I enjoyed the show and went to an have an early night's sleep. Only in Chiang Mai I managed on getting more than 5 hours sleep at night. 
Classic House Massage  (#1) - The next day after visiting an elephant sanctuary, I put my daughter to sleep and walked 7 minutes from Le Meridien to Classic House Massage. I was really pleasantly surprised by the variety of men that were there. About 20 guys on display with a fair amount of guys on the beefy / muscular side. Also liked the clarity of the place: on the menu it is written 600 for massage, 600 minimum tip and a recommendation of 1000 minimum tip if happy ending services were supplied. Massage was very erotic, and the end was indeed happy. The day after I came again to Classic (#2) and it was exactly as enjoyable with a different and attractive guy. 
Ram Bar - Second night in Chiang mai went to the bar there and enjoyed the show which was also professional and entertaining.

Most of my Chiang Mai time was spent in attractions: Art in Paradise, Umbrella factory, markets, Zoo etc. I enjoyed the calmness of that place after the intense weekend in BKK. 
After 3 nights there it was travel time again to our next leg - a new destination for me as well - Singapore! 
 

Day 5: Caught the BTS to Asok and had a wander around Terminal 21. Ended up buying some jeans, shorts, and shirts from H&M and had a cheap lunch at the food court, although the serving sizes were quite small. When I got bored, I went back on the BTS to Sala Daeng and went to Prime Massage. I've heard a lot of good reviews about this place so I decided to give it a try. When I got there, they were quite busy, but it was quite efficiently run. I wanted to get a 1 hour foot/1 hour Thai massage, but the lady at the counter said if I got a 30 min foot/1 hour Thai then I wouldn't have to wait, and could get started straight away. So I went with that. 450 baht. My guy was a cutie, and quite smiley as well. Foot massage was good, and the Thai massage was amazing - out of all the Thai massages I've had in my life, I don't think I've ever been contorted into so many positions. He was really strong and worked hard, so I tipped him 300 baht. Since I was quite impressed, I made an appointment for the following day with the same guy for a 1 hour foot/1 hour Thai. On the way back to the BTS station I took a detour and walked through the Patpong night market (it wasn't quite night yet so they were still setting up), and then back down the street that some of the bars are on (Screwboys, Dreamboy Paradiso etc). I also walked past so many massage places. I just want to try them all out!
Day 6: Went to MBK to have a bit of a look, nothing too spectacular there, lots of stalls like a market. I had some lunch and bought a pencil case. Afterwards I went back to Prime for my appointment, got there a little earlier and was able to start straight away. My massage guy looked like he got a fresh haircut too. If I thought yesterday was good, today was even better and he was using some moves that he didn't use yesterday. Pretty legit massage, although he came in contact with my 'area' a few times, whether accidental or deliberate, I didn't mind. Day 7: So today was my last day. I decided to move hotels as my flight was tomorrow and I didn't want to travel on the BTS during rush hour with my suitcase (plus I wasn't super into my current hotel). I ended up booking a room at Sukhon which is literally right outside the Phaya Thai BTS station. It's a relatively new hotel (opened last year I think), and it was clean and neat. I was on the 5th floor (top floor) and had views of the BTS station lol. Ganymede is quite close by so I decided to pay a visit. There were about 6 guys there on arrival and I chose a slightly muscular one (not sure why, as I'm more into the twinky types), but I did like his face. Showered alone down the corridor, went back to the room and the guy came in after a while. Massage was okay, was offered sex and asked him to f*ck me but he had trouble staying hard. Day 8: The hotel provided a choice of American or Thai breakfast and I got the Thai option and it was quite delicious. Shortly afterwards, I walked to the airport link station (so close) and finished my time in Bangkok (I would then go to spend some time in Singapore then Bali).
So all in all my trip was okay, I could have done a lot more (I just got really complacent and lazy), but I learn more every time I come here. I think for next time I'll stick to a bit of an itinerary so I can get stuff done, and in the meantime work on my health a little bit so that when I actually arrive, I'm not so tired all the time. I did get some offers from forum members to meet up, but I didn't end up arranging anything - so apologies and perhaps next time! I also didn't manage to go to any bars, to be honest I'm a little nervous, but I guess once I do it, it'll be easier and I won't be so scared! I have quite a bit of Thai baht left so I'm actually thinking about coming back in December and spending some time in Bangkok and Pattaya. I've already been looking at flights. I don't have much on during that time so I thought I may as well.. thank God we humans are very adaptable to changing circumstances.
Travel expenses can be reduced  by being more diligent looking for better deal, booking early , travelling  mid-week, gnashing teeth in economy class , going for less convenient connections etc.
With abundance of rooms in BKK and Pattaya hotels there always will be good deal on accommodation although we may settle for less luxurious than our  usual options.
When comes to food  as you noticed we have great scope for savings  by  dining in cheaper places and on the street. What we may reduce if necessary is social dining, in my case watching where I'm inviting my offs to have their meals .
Even in entertainment we can  recuse out spending by resorting from time to time to cheaper places ( two massages at Senso or Prince = three of the same length at Bangkok Massage or Arena), replacing LT with ST from time to time or even giving ourselves whoring break every so often. Being loyal to particular guy may give  us scope to look for deal on tip from him.
Keeping  cool head in bar helps too , one drink is enough, if we are offing guy known to us no need to buy him a drink and does mamasan really deserve that 100 baht tip just because she smiles  at us? 
Shortening trip be few days may help and reducing number of annual trips may produce some savings and above everything setting realistic budget and sticking to it my help - worked for me on last trip , first in time  immemorial without trip to ATM.

Leg 2 - Pattaya - ALL inclusive hotel   Arrived Pattaya mid week Monday till Friday.   Hotel review - Centara Grand Mirage - 2 interconnected rooms (150 USD per room per night): This hotel is for families. I see no reason for a gay solo/couple coming here without children. It is far north and kinda isolated. For my needs it was great because this hotel has children activities, water park, children's pool etc. They also guaranteed me in advanced the interconnecting rooms which was the most important thing for me because I wanted privacy at night but also to be near my daughter and nanny. Breakfast is OK, but not more than that. Hotel Gym is one of the best I have seen for a hotel.    Grindr hookup #3 - same as in Bangkok, I fakedGPS to the hotel a week before arriving. I then realized that one of the staff members is hot and gay. We first met on my arrival day but just for drinks cause he had a day off and was headed to BKK for the night. So 3 out of 4 of my nights there, my night fun was also provided by the hotel. So, I had zero intrest in leaving to explore out Pattaya. Fun with that guy was awesome with great chemistry and sex so meeting him night after night was something I was anticipating. This hotel was really ALL INCLUSIVE for me.    Pattaya night out - I went out on my first night cause that was the only night my hotel entertainment was out of town. Saw a really shredded guy outside Scandic offering me a massage. He stated the price of 1500 THB which seemed high for Pattaya but I was horny so didn't care. I scheduled it so we'll be in fun to party places during weekends (Bangkok, Singapore) and the mid week we went to places where it doesn't matter that much if you come on weekends or not (Pattaya, Chiang Mai).    Since this trip was relatively long, and most of my time was spent at kids stuff with my daughter, this report will not be a detailed one. I'll try to stay focused on things that will be of interest to this forum. Take into consideration that my trip was about 70% child play and 30% dad's play haha. The report will focus on dad's play.   Leg 1 - BKK weekend - Engines on   TLV-BKK 11h flight went easy considering the fact that it was just me, 3yo girl and a fair amount of carry on luggage. When traveling with family, the concept of serviced apartments is my preferred. Apartment was all I could ask for: big, clean and each room had a lock haha. Breakfast was medium. Will return.    I decided that first day of my trip will be dedicated to commercial sex and the days after will be dedicated to non commercial sex.    Prince #1 -  My goal with fasttrack was to arrive to the hotel at 4PM so my daughter will nap immediately upon arrival. I've become an expert in adjusting her naps to my needs haha.
