Part 1: Traveling to and arriving in Bangkok
 
 
It was last February I decided to start planning and booking my trip.
Qatar Airways was having great deals with free cancellation option. I didn’t want to go to Thailand using any Sandbox- or Quarantine-scheme. Too much of a hassle for me. I thought let’s book the flights for October as I was thinking by than life will be almost “back to normal” again. And when for some reason that would not be the case, I could cancel my flights and get a 100% refund.
It was before fuel prices went up and Russia invaded Ukraine. So I was lucky to be able to book business class seats from Amsterdam to Bangkok for only 750 euro per way, so in total it was around 1500 euro. In my opinion, Qatar has one of the best Business Class products in the world. Q-suites. Maybe even the best. Both my Amsterdam-Doha and the Doha-Bangkok flight were planned to be flown with planes
that are equipped with Q-suites. So much space and privacy! In my opinion this is almost first class, with your own little cabin that can be closed for ith a sliding door. Love it. And it’s so nice to be able to have a good sleep in flight and arrive in Bangkok relaxed and not too tired.

It was just after I checked in for my flights that I received an email fro Qatar Airways. The aircraft type of the flight Doha-Bangkok would because of "operational reasons" be changed and not have Q-suites on board. Actually, when you Google search, you will find out many complains about this and it seems Qatar Airways is doing this regularly. I was very disappointed that on my 2nd flight I would be in their old Business Class product in a 2-2-2 configuration. Less space, less confort, less privacy. It was a night flight. So they dimmed the cabin nights and actually the entire business class cabin went to sleep. So I was able to get some decent rest during the flight in the end. And the food was still very good, as it was on my first flight that did have the Q-suite expierence.

My flight touched down in Bangkok just after 1pm. The long walk to immigration followed. I could use the fast-lane and there was only 1 other traveller waiting before me. But also the normal waiting line at immigration for economy class passengers was not long at all. What a difference with pre-covid. I remember extreme long queues there. 
Within 1hr after arriving, I finished immigration, got my luggage, taken some bahts from the atm, bought and activated a Thai simcard and was on my way to Silom in a taxi. The driver asked me 600 baht including tollway, not using the meter. Thats seemed a fair price for me. As Bolt was charging 500 baht without toll fee. 
I decided not only to spoil myself with the flight, but also to stay in Le Meridien hotel for the first 3 nights in Bangkok. Let’s say this was my way of celebrating my return to Bangkok… Same as for my flights, I was able to get a very good deal and ended up paying only 75 euro per night.

Smooth check-in, beautiful room with bath and seperate shower and view over the city on the 17th floor. You need a keycard to activate the elevator. So can't invite any boys into the room without getting them from the street / reception yourself. That's a small minus for me. That being said, this does NOT mean it's a good idea for men without symptoms to get PSA levels checked on some sort of routine level. I would urge board members reading this to get their health advice from public health officials whose job it is to evaluate all of the scientific facts, instead of relying on emotional stories or discussions.

Benefits of Early Detection and Treatment
The goal of screening for prostate cancer is to identify high-risk, localized prostate cancer that can be successfully treated, thereby preventing the morbidity and mortality associated with advanced or metastatic prostate cancer.
Adequate evidence from randomized clinical trials (RCTs) shows that PSA-based screening programs in men aged 55 to 69 years may prevent approximately 1.3 deaths from prostate cancer over approximately 13 years per 1000 men screened.3, 4 Screening programs may also prevent approximately 3 cases of metastatic prostate cancer per 1000 men screened.3 Current results from screening trials show no reductions in all-cause mortality from screening. There is inadequate evidence to assess whether the benefits for African American men and men with a family history of prostate cancer aged 55 to 69 years are different than the benefits for the average-risk population. There is also inadequate evidence to assess whether there are benefits to starting screening in these high-risk groups before age 55 years.
Adequate evidence from RCTs is consistent with no benefit of PSA-based screening for prostate cancer on prostate cancer mortality in men 70 years and older.
Harms of Early Detection and Treatment
The harms of screening for prostate cancer include harms from the PSA screening test and subsequent harms from diagnosis and treatment. Potential harms of screening include frequent false-positive results and psychological harms. One major trial in men screened every 2 to 4 years concluded that, over 10 years, more than 15% of men experienced at least 1 false-positive test result.5 Harms of diagnostic procedures include complications of prostate biopsy, such as pain, hematospermia (blood in semen or ejaculate), and infection. Approximately 1% of prostate biopsies result in complications requiring hospitalization. The false-positive and complication rates from biopsy are higher in older men.3 Adequate evidence suggests that the harms of screening and diagnostic procedures are at least small.
PSA-based screening for prostate cancer leads to the diagnosis of prostate cancer in some men whose cancer would never have become symptomatic during their lifetime. Treatment of these men results in harms and provides them with no benefit. This is known as overdiagnosis, and follow-up of large randomized trials suggests that 20% to 50% of men diagnosed with prostate cancer through screening may be overdiagnosed.3 Overdiagnosis rates would be expected to increase with age and to be highest in men 70 years and older because older men have high risk of death from competing causes.
Harms of prostate cancer treatment include erectile dysfunction, urinary incontinence, and bothersome bowel symptoms. About 1 in 5 men who undergo radical prostatectomy develop long-term urinary incontinence requiring use of pads, and 2 in 3 men will experience long-term erectile dysfunction. More than half of men who receive radiation therapy experience long-term sexual erectile dysfunction and up to 1 in 6 men experience long-term bothersome bowel symptoms, including bowel urgency and fecal incontinence.3 Adequate evidence suggests that the harms of overdiagnosis and treatment are at least moderate.
Adequate evidence shows that the harms of screening in men older than 70 years are at least moderate and greater than in younger men because of increased risk of false-positive results, harms from diagnostic biopsy, and harms from treatment.
Before consenting to screening (which is different from testing when there are symptoms), it's important to be aware of both the potential benefits of screening (which, if you read the USPSTF, are quite minimal), as well as the potential harms--which are numerous and much more likely. So, yes, the original poster should probably get a PSA done due to his symptoms (especially if they're obstructive), and I hope he keeps us updated. However, the takeaway message should NOT be that all men over 50 should rush in and get this testing. There are some pretty significant harms that can come from screening, especially since the vast majority of prostate cancers never affect the life of the  man who has it. There is a massive amount of science which has looked into this question, and professionals have spent large amounts of time actually reviewing the data.

Well this is my last day in Brazil.

 To start, I’m glad I even got here.   With omicron seemingly everywhere I was concerned about getting a negative test for the flight.   Equally concerning is traveling with so many flight cancellations left and right.    To recap what others have said, I needed proof of vaccine, a negative Covid test within 72 hours (PCR test) or an antigen test within 24 hours, a health document filled out and printed within 24 hours as well.     The entire process was smooth on American Airlines part and when I arrived at passport control in Rio he just causally glanced at my vaccine card but didn’t ask for any other documents.   
 
I haven’t traveled internationally in a couple of years and I feel rusty.   I didn’t account for my flight arriving at 730 am and my Airbnb wouldn’t be ready till 3pm.   So I ended up booking a hotel room in Copacabana.   I should have booked the entire previous day to ensure my room was ready.  But it’s all good.    I took Uber from the airport.   One curiosity I’ve had to learn (at least from the airport context) is communication with your driver prior to pickup.   They need confirmation or something via text.   There was miscommunication and I had a few drivers drop the transaction until a lovely lady picked me up.   Uber prices are cheap so I went “upscale” and booked Uber black because it was apparent the moment I exited the doors that winter was behind me and I needed air conditioning.    For what it’s worth my Uber black car was just a non old Honda Civic.  Perfectly capable and acceptable for the price though.

1. PCR tests
For Canadians visiting Brazil, PCR tests are required both for flying into Brazil and back to Canada. It's annoying that you have to add these into your trip plan. The GIG airport PCR testing is very organized and results are available in 4 hours (3 for my experience). Cost is 315 reals. They will help you print a copy as well at the GIG basement level where you do the test. The fee is insignificant comparing to the hassle that you have to plan it and the uncertainty that one might test positive.
    
2. VIP lounges
Very disappointing, GIG VIP lounges are closed to priority club or lounge pass users. only business class travellers.
At Panama city, lounges are all open with reduced food options, much better.
3. Entry forms online before boarding
You need to fill in corresponding government forms before you can board the flight. So if you didn't go through the counter (used web-checkin) and didn't find the form yourself online, make sure you ask staff for the link before boarding time. I had to scramble to fill in the Brazilian form in order to board with just a few minutes to spare before they closed the boarding gate.
4. hotels
other than you are expected to put on a plastic glove when fetching food, not much changed. One hotel I stayed in will do cleaning only on demand, which actually works better for me. Copacabana Hotels are in high demand on weekends. Even though it's cold and rainy for most of the last 2 weeks I was there.
5. 202
Lot of boys. Notably few foreign tourists. None of my favourites from 2019 trips still work there😒. However, did find two new boys I like, which is very good. I usually go with Claro, which is running a promotion of 30Reals for 30 days with 12GB data. I asked the slim white twink from 202 to activate the sim for me. In return, I saw him twice and bought him Sushi. I usually add credit with cash in one of the Americana stores.
